Hypnosis for fibromyalgia and widespread pain

Fibromyalgia is the clearest example there is of pain generated by a sensitised nervous system rather than damaged tissue — which makes it, in principle, exactly the kind of problem hypnosis is built for.

Being believed, first

Most people with fibromyalgia have spent years being disbelieved. Normal bloodwork, clear imaging, a succession of clinicians implying — sometimes saying — that it is stress, or low mood, or something you are doing to yourself.

So let me be unambiguous. Fibromyalgia is a recognised condition with a documented neurophysiology. Central sensitisation, altered pain processing and disrupted sleep architecture are measurable. You are not making it up, you are not exaggerating, and the absence of a lesion on a scan proves nothing at all about how much you hurt.

That is not a bedside manner point. It matters clinically: people who feel dismissed brace harder, and bracing amplifies pain.

Why the sensitisation model makes hypnosis relevant

If pain came only from damage, fibromyalgia would be inexplicable. It is explicable as a gain problem — the volume control on the whole system set too high, so that ordinary pressure, ordinary fatigue and ordinary temperature changes all register as pain.

Hypnosis operates precisely on gain: on attention, on threat appraisal, on how a sensation is interpreted, and on the arousal level of the whole system. That is a good match for the mechanism. It is not a cure — nothing currently is — but it addresses the right layer.

The three things we usually work on

1. Sleep

Almost everyone with fibromyalgia sleeps badly, and the relationship runs both ways: poor sleep measurably lowers pain thresholds the following day. Sleep is also one of the things hypnosis is most reliably good at. For that reason I very often start here, even though it is not what people come in asking for. Improving sleep tends to move everything else.

2. The flare cycle

Good day, do too much, three bad days, do nothing, deconditioning, guilt, repeat. Breaking this boom-and-bust pattern requires changing an emotional relationship with activity — the fear of the crash and the guilt of the rest — not just a pacing schedule you already know about and cannot stick to.

3. Whole-body arousal

Fibromyalgia rarely arrives alone. IBS, migraine, TMJ, restless legs, sensitivity to noise and light and temperature — these cluster because they share a common driver. Lowering baseline nervous system arousal is slow work, but it is work that touches all of it at once rather than chasing symptoms individually.

An honest word about expectations

I am not going to tell you hypnosis cures fibromyalgia. There is research supporting hypnosis for fibromyalgia symptoms, and there is also research showing that pooled effects for chronic pain conditions are more modest than enthusiasts claim. Both things are true.

What I will say is that hypnosis has no side effects, does not interact with anything you are taking, gives you a skill that remains yours, and targets a mechanism that is genuinely central to this condition. For a diagnosis where most people have been offered very little that works, that is a reasonable thing to try — with clear eyes about what it is.

Questions about fibromyalgia

Will hypnosis help the fatigue as well as the pain?

Often, indirectly and substantially — mostly by improving sleep quality and by reducing the enormous energy cost of holding a body in constant guard. Several clients have told me the fatigue shifted before the pain did.

I have brain fog. Can I even do hypnosis?

Yes. Hypnosis asks you to relax and follow a voice, which is far less demanding than most things you do in a day. Fog is not a barrier, and for some people the deep rest of trance is one of the few things that lifts it temporarily.

Does this work for ME/CFS or long COVID too?

There is overlap in mechanism and I do work with people carrying those diagnoses, particularly on sleep, pacing and arousal. But post-exertional malaise needs careful handling and I will not push activity in a way that risks a crash. The evidence base here is thinner and I will say so.

How many sessions realistically?

For fibromyalgia I would ask you to plan on eight rather than six, because we are working on baseline arousal rather than one location, and that takes longer to shift. The daily recording matters more here than anywhere else.
